Please print/fill out and submit the Activity Fee Form to the LVMS Office (if in grades 7/8) or the LVHS Office (if in grades 9/10/11/12).  DO NOT submit the form and/or payment to coaches or the Athletic Director.  Copies of the form are also available at each school office.
